Working as part of the overall Camden and Islington Healthy Lifestyles programme for lot 2 we are seeking a provider who can deliver NHS Health Checks and Community Outreach both as part of the overall lifestyle programme and integrated with the Hub (Lot 1) NHS Health Checks is a nationally mandated public health programme that aims to keep people well for longer. It is a risk assessment and management programme that aims to prevent or delay the onset of major non-communicable disease including heart disease, stroke, diabetes, and kidney disease through earlier awareness, assessment, and management of risk factors and conditions.
