Prevention and early intervention services are a key element in the Council?s response to managing this challenge. The Council and Islington Clinical Commissioning Group (ICCG) already provide a mainstream re-ablement service to people coming out of hospital which is aimed at reducing reliance on on-going support from statutory services.
