Acute Home Visiting Service for NHS Basildon & Brentwood CCG

Basildon and Brentwood Clinical Commissioning Group ("The Contracting Authority") is seeking suitably qualified and experienced providers to supply a 7-month Acute Home Visiting Service. The Acute Home Visiting Service (AVHS) will work with member GP practices to support the provision of home visits for vulnerable housebound patients. The provision of support in this area is in-line with the relevant GMS/PMS clauses relating to Home Visiting requirements.
