Provision of an Integrated Care Centre Service for patients in Herne Bay. The overall aim of the service is for the provision of an Integrated Care Centre for patients from the Herne Bay area to provide: - a locally accessible integrated model to include access for minor injury/illness including x-ray diagnostics, enabling easy and timely access to urgent health services, ensuring patient safety - access to a range of community based services to include DVT, catheter care, wound clinic, musculo-skeletal services, and frailty clinic.
