Care and Support at Home - Short Term Intensive Reablement Services

The emphasis of the Reable Somerset service is to coordinate and join up all care and support, and use local information and resources for successful reablement. The outcome will be helping people in a different way, so that where possible, needs are reduced. This coordinated approach to care, health and community services allows for good interaction between professionals and people eligible for the service. It will identify local community based and voluntary resources to aid person centred planning and shared decision making.
