The facility would be required to accept step down patients from acute hospitals and step up from community services and/or primary care. The patients would be supported by 24 hr nursing care of CLCH Nurses and regular medical ward rounds. Therapy services would be provided daily for rehabilitation of physiotherapy and occupational therapy goals, other AHP services would be accessed as required and delivered by CLCH community services.
