Community Crisis Support Service in Nottinghamshire

The crisis support service will provide intense and focused health and social care (including personal care) to assist people through a worsening health crisis to remain living in their own home and maintain independent living skills. The key elements are a rapid response to the individual's home, a thorough assessment and an integrated care package allowing for short term results and a long term solution.
