The Royal Borough of Kingston upon Thames is seeking proposals for the provision of the Community Outreach NHS Health Check Programme. The overall aim of this service is to improve the health outcomes and the quality of life of the eligible population by providing health checks in easily accessible community settings. This will support the delivery of the programme in Primary Care by providing a flexible health check service that can be accessed opportunistically.
