Connecting Communities - Mobile Social Prescribing Services

Suffolk County Council is seeking a delivery partner for social prescribing services. The Council is part of an EU funded programme to develop local community-based services to improve the health and wellbeing of older populations affected by rural and social isolation and deprivation. The programme will create a mobile social prescribing 'plus' scheme which will have the same components as a traditional social prescribing scheme but will be sited within a wider framework of early intervention and prevention services.
