Consultant Led Acute Medical Care at Home

Provision of medical and nursing care to patients in their own home as an alternative to a hospital admission. The external Provider will work with Emergency Department (ED) consultants to identify patients who would normally be admitted to an acute hospital bed and manage their condition at home. The patient will receive the treatment they would have had in hospital and this may include, monitoring, IV antibiotics, medications and rehabilitation therapies.
