Dementia Support Service on behalf of NHS Wandsworth CCG

Invitations to tender are now welcome from suitably experienced and capable providers to deliver the provision of a Dementia Support Service. WCCG has developed a new service model which will play a central role in providing systematic and proactive support to people and their carers/family who are living in the community with dementia through personalised case management.
