NHS Kernow Clinical Commissioning Group is seeking responses from organisations that can provide; a) A software package that delivers a Digital Shared Care Plan containing essential patient information, including preferences for their palliative care and be easily accessible and updatable by the patient and the professionals involved in their care; and b) A platform for an Electronic Palliative Care Co-ordination System that delivers an end of life (EOL) register and that enables the sharing of the register by care professionals.
