The service will provide both early intervention services and prevention of readmission. The service will be provided by 2 x 0.8 WTE Mental health wellbeing recovery practitioners and 1 x Activity coordinator to provide a coordinated programme of support and fill resource gaps and provide support to the Rehabilitation team within TEVWS Durham sites.
