The contract was awarded for 2 years with the option to extend for an additional year. This contract was awarded following an open procurement. In order to develop the falls and dementia service further the CCG wishes to pilot a social prescribing service and care coordination service in relation to this cohort of patients. This service can only be delivered by the existing provider as it needs to be embedded with the existing service.
