Health and Social Care Connect (HSCC) provides a single access point into adult community health and social care services in East Sussex, enabling adults in need of care and support to access the right service at the right time. The Clinical Nurse Advisor Service will have a central role in assisting people to access information and services in a joined up way through the provision of clinical advice within an integrated HSCC service.
