Provision of Care Planning Training for General Practices in the Hastings and Rother area of East Sussex. The objective of the service is to: - Provide GP practice staff with personalised care planning approach training - Ensure the development of motivational interviewing skills - Ensure a full understanding of the care planning ethos and patient empowerment.
