Fast Track Home Care procurement (Pilot) - North Staffordshire and Stoke-on-Trent CCG's

North Staffordshire and Stoke on Trent Clinical Commissioning Groups are looking to commission, as a pilot for 12 months, a different way of commissioning home care packages for adult patients eligible for Fast Track Continuing Health Care services across Northern Staffordshire. For those patients with a rapidly deteriorating condition that may be entering the terminal phase, the Fast Track process was set out in the National Framework to enable immediate provision of NHS Continuing Health Care.
