Health Improvement IT System

Kent Community Health NHS Foundation Trust are seeking a provider of a Kent Health Improvement Service IT system which will record all outcomes from clients wanting to access a service to support a behaviour change. This will include recording of an initial assessment, to booking an appointment and navigating the client to a service that is accessible to them, recording of further assessments once at the service entry point, recording of follow up appointments and to measure long term behaviour change outputs to have a function to record outcomes at 6 and 12 months after sign off from the service.
