Home With Support (East Kent)

Kent Community NHS Health Foundation Trust (KCHFT) is seeking tenders from suitable experienced providers who can deliver a Home with Support Service. The Home with Support Service supports the discharge of patients who have been assessed in the Acute as safe to go home by receiving a further assessment of need, rehabilitation and a period of recovery at home and develop a plan of care and onward referral if required. This service is an extension of the KCHFT Rapid Transfer of Care Service, and will work in collaboration with Rapid Response, Acute Response Team, Rehabilitation teams to support patients with complex need to be transferred home from either the Accident and Emergency Dept. …
