Newham CCG is planning an ambitious transformation of its community health services over the next 5 years. As part of our next steps and following on from your feedback, a second provider engagement workshop will be held on 9 June 2016 to share with you the work which the CCG has done so far on: •The design of the future Care Model •Innovative Contracting Models for Community Health Services •Setting practical and realistic outcomes.
