Kettering General Hospital Healthcare at Home Service

The service will provide individualised sub-acute healthcare to patients in their own home who are not medically fit for discharge, but who are clinically stable and can have their care delivered in a less acute setting. Care will be delivered by a range of registered and unregistered healthcare professionals, working closely with hospital physicians, surgeons, therapists, nursing staff and other community health and social care practitioners to deliver interventions typically restricted to acute hospital settings.
