The funds will support intervention work; for example, extended consultations for identified people where a targeted intervention is appropriate or regular extended multi-disciplinary team meetings to support integrated working. As a pilot LCP you will be in a strong position through any initial intervention testing to support 'Fast Follower' LCPs to take forward tried and tested intervention methods.
