The Council requires a Managed Service for the Completion of Social Care Reviews (the Review); these will predominantly be reviews 6-8 week post care package commencement, although it is envisaged that the Provider may be required to undertake other reviews, at different timeframes. Reviews will be undertaken to a standard which is Care Act 2014 compliant. This may be the first opportunity to review a newly implemented package of support.
