New Care Models - Communities of Practice

Communities of Practice can be a highly effective enablers of profound change - offering a foundation of network from which people can build learning capabilities, enhance personal results, create new business practices and deliver enhanced business results - for patients, staff and communities. In 2016/17 communities of practice were held across the new models of care, multiple cohorts. These new models of care communities of practice allowed sites to access peer support, alternative views and perspectives and access tools.
