Phase 1 of the pan-London NHS continuing healthcare domiciliary care AQP will be open for applications between 13 July and 21 August 2015 at www.supplying2nhs.com. Successful providers will be added to a domiciliary care AQP list which is used by NHS Clinical Commissioning Groups (CCGs) and discharge teams to make continuing healthcare placements. The AQP will be the standard route of commissioning for the participating CCGs. This means that domiciliary care packages will be commissioned with AQP providers.
