NHS Hambleton, Richmondshire and Whitby CCG End of Life Services

HRW CCG intend to commission a Domiciliary Care Service for Hambleton and Richmondshire. The service described within this specification forms part of the integrated end of life pathway for the Hambleton and Richmondshire locality only. A rapid relief team will support the community team to deliver end of life care to patients at home. This team will consist of Health Care Assistants (HCA's) which will be centrally co-ordinated and delivered.
