North Staffordshire Community Falls Coordinator Service

North Staffordshire and Stoke on Trent CCGs wish to commission a community based falls coordinator service for the North Staffs and Stoke on Trent CCG population. The community based falls service will be expected to reduce individuals falls risks, improve patients' health outcomes and support people to live independently for longer. To minimize the harm due to falls and the number of associated hospital admissions there is a requirement to focus on prevention.
