NHS Nottingham City CCG seeks to appoint a suitably qualified provider to deliver high quality therapy led community based specialist rehabilitation for patients with any advanced or progressive illness, regardless of diagnosis identified with palliative or end of life care needs to achieve the following outcomes: • Patients experience increased independence following intervention. • Patients are maintained in their own home environment (where appropriate). • A reduction in unplanned hospital admissions. • A more co-ordinated approach between community and secondary care services. • Improved experience and choice to patients at end of life.
