Provision of Support for the Development of the Place Based Plan for Hull

Hull City Council and Hull Clinical Commissioning Group want to develop a common vision and strategy that embraces all of the local public sector organisations in Hull and is connected to the population we service. We need a radical upgrade in prevention and public health and to embed prevention within the City Plan to address the wider determinants of health. We require skilled support to develop and implement the Place Based Plan.
