To support a patient and their carer in their home or care home setting, for a maximum of a 48 hour period or 72 hour period over a bank holiday period, during what is perceived to be the final six weeks of the patient’s life or for a patient with a life limiting condition when their caring situation is considered untenable by a health care professional until an appropriate care package can be arranged.
