Social Prescribing for NHS Warwickshire North and NHS Coventry & Rugby CCGs

NHS Arden & GEM CSU on behalf of NHS Coventry and Rugby and NHS Warwickshire North CCGs is oinviting suitably qualified and experienced providers to express an interest in delivering a Social Prescribing Service across the Coventry & Rugby and Warwickshire Norht areas and a Link Worker Service in the Warwickshire North area. The NHS Long Term Plan outlined its intentions for the roll out of the NHS Personalised Care model across the country, reaching 2.5 million people by 2023/24 and then aiming to double that again within a decade.
