Specialist End of Life Home Care (Domiciliary Care) Support on Behalf of NHS Stockport Clinical Commissioning Group

There is an identified need to provide high quality domiciliary care in the specialist area of End of Life. This will support clinical need in the patient's own home, whilst acknowledging the need for sensitive and holistic care. The service will provide a community based End of Life domiciliary care service for patients with a Stockport GP who require a swift discharge from hospital or increase in care at home due to a rapidly deteriorating condition that may be entering a terminal phase; the service will be required to have a safe package of care ready to commence within 24 hours, 48 hours as a maximum.
