WCC - Discharge to Assess Pathway 2

Discharge to Assess (D2A) is designed to reduce unnecessarily prolonged acute hospital stay. It is a multi-agency approach between the Authority and SWFT including Continuing Health Care, which allows assessment for ongoing services to take place after discharge from hospital. There are three pathways within D2A: Pathway 1: For customers who can return home with additional support from CERT or Reablement. Pathway 2: For customers who cannot be discharged directly home but who have the potential to do so if given additional rehabilitation.
