Wiltshire CCG - Ophthalmology Services

NHS Wiltshire CCG wishes to procure community-based Ophthalmology Services for service users registered with a Wiltshire GP. It is envisaged that the service will assess, diagnose and treat a number of conditions and provide a Referral Refinement service to ensure appropriate referrals are made to secondary care. The service will assist in the management of long term conditions and supported self-management for patients with such conditions.
