A 16 bedded unit is to be developed as part of a sub acute community pathway. This is to meet the needs of complex older people. The patients would be admitted either via the community sub acute pathway from home (Enhanced Care Team/GP) or from the Acute Trust following inpatient acute care.The unit would be subject to all quality governance, performance and finance controls currently in place within the Trust.
