For the Provision of an Innovation Partnership to Co-Design and Deliver a Social Prescribing Network

Oldham Council, on behalf of the Oldham Cares Alliance, is embarking on a project to co-design, establish, deliver and further develop the Social Prescribing Network and are now looking to appoint a partner(s) to work collaboratively with Oldham Cares innovatively, to co-design, establish, deliver and further develop this provision across the borough of Oldham. It is anticipated that the successful partnership will deliver the social prescribing network in all clusters, whilst developing the pathways and systems in partnership with Oldham Cares in order to roll out the delivery across all of the clusters in the Borough.
