West Sussex County Council Reablement Services Introduction The provision of reablement services across West Sussex to support customers following a change in ability, possibly caused by a medical condition. The intention of the reablement service will be to re-skill the customer to enable them to retain or regain their independence and to prevent or reduce the need for longer term support. It can be provided to a customer following a community based assessment or upon their discharge from hospital.
